dc.description.abstractAccurate delineation of the Gross Tumor Volume (GTV) is a critical step in radiotherapy planning for Non-Small Cell Lung Cancer (NSCLC), yet manual contouring by radiation oncologists is time-consuming and subject to significant inter-observer variability. This, thesis presents an automated GTV segmentation pipeline based on a three-dimensional U-Net (3D U-Net) architecture with 5.6 million parameters, trained and evaluated on 65 patients from the publicly available NSCLC-Radiomics dataset (The Cancer Imaging Archive). After preprocessing (Hounsfield unit normalization and tumor-centered cropping to 96×192×192 voxels), the network was trained with a combined Dice and Weighted Binary Cross-Entropy loss (positive class weight = 100) to address the severe class imbalance inherent in tumor segmentation, on a single NVIDIA Tesla T4 GPU (Google Colab) in approximately 11 minutes. On an independent test set of 10 patients, the model achieved a mean Dice Similarity Coefficient (DSC) of 0.547±0.228 (best individual result 0.856). A strong positive correlation was observed between tumor volume and accuracy: large tumors (≥ 100 cm3 ) reached a mean DSC of 0.750, comparable to published results, while small tumors (< 20 cm3 ) yielded only 0.259. The model exhibited high Sensitivity (0.855) relative to Precision (0.474), favoring over-segmentation—a clinically preferable behavior in radiotherapy planning. A Test Time Augmentation experiment with spatial flipping produced a negative result (DSC decreased to 0.538), attributed to the disruption of anatomical laterality cues in the thorax. These results demonstrate the feasibility of training a competitive GTV segmentation model under limited data and computational constraints, and contribute a detailed size–performance analysis often reported only in aggregate in the literatureen_US
dc.description.abstractيُعدّ التحديد الدقيق لحجم الورم الإجمالي (GTV) خطوةً محوريةً في تخطيط العلاج الإشعاعي لسرطان الرئة غير صغير الخلايا (NSCLC)، غير أن التحديد اليدوي الذي يُجريه أطباء الأورام الإشعاعية يستغرق وقتاً طويلاً ويخضع لتباين ملحوظ بين المراقبين. تُقدِّم هذه الأطروحة منهجية آلية لتجزئة GTV تستند إلى بنية U-Net ثلاثية الأبعاد (3D U-Net) تضمّ 5.6 مليون معامل، تم تدريبها وتقييمها على بيانات 65 مريضاً مستقاةً من مجموعة بيانات NSCLC-Radiomics المتاحة للعموم (The Cancer Imaging Archive). بعد مرحلة المعالجة الأولية المتمثِّلة في تطبيع وحدات هاونسفيلد وقصّ الصور بمحاذاة الورم لتبلغ أبعادها 96×192×192 فوكسل، جرى تدريب الشبكة باستخدام دالة خسارة مُركَّبة (Dice ووزن نظير الانتروبيا الثنائية، بوزن 100 للصنف الإيجابي)، بهدف معالجة الاختلال الحاد في التوازن بين الفئات الكامن في تجزئة الأورام. نُفِّذ التدريب على وحدة معالجة رسومية واحدة من طراز NVIDIA Tesla T4 عبر Google Colab في ما يقارب 11 دقيقة. على مجموعة اختبار مستقلة مؤلَّفة من 10 مرضى، بلغ متوسط معامل تشابه دايس (DSC) 0.547 ± 0.228 (أفضل نتيجة فردية: 0.856). لوحظت علاقة إيجابية قوية بين حجم الورم ودقة التجزئة: إذ بلغ متوسط DSC لدى الأورام الكبيرة (أكبر من 100 cm³) قيمةً قدرها 0.750، وهي نسبة مماثلة للنتائج المنشورة في الأدبيات، في حين لم تتجاوز الأورام الصغيرة (أقل من 20 cm³) قيمةَ 0.259. أظهر النموذج حساسيةً مرتفعة (0.855) مقارنةً بالدقة (0.474)، مما يُشير إلى ميل نحو التجزئة المفرطة — وهو سلوك أكثر أماناً ومقبوليةً سريريةً في سياق تخطيط العلاج الإشعاعي. كما أسفرت تجربة تعزيز بيانات وقت الاختبار (TTA) باستخدام الانعكاس المكاني عن نتيجة سلبية (انخفض DSC إلى 0.538)، ويُعزى ذلك إلى اضطراب إشارات التوجه التشريحي الجانبي في منطقة الصدر. تُظهر هذه النتائج إمكانية تدريب نموذج تجزئة GTV تنافسي في ظل محدودية البيانات والقيود الحوسبية، وتُقدِّم تحليلاً مُفصّلاً لعلاقة الحجم بالأداء، وهو ما تكتفي الأدبيات السابقة عادةً بعرضه بشكل إجمالي دون تفصيل.-
