Abstract: Introduction : La cholécystite aiguë lithiasique est la complication la plus fréquente de la lithiase vésiculaire. La cholécystectomie laparoscopique constitue la technique de référence dans le traitement de la cholécystite aiguë. Cependant, le moment optimal de l’intervention reste controversé. L’objectif de cette étude est d’évaluer l’impact du délai opératoire sur les résultats de la cholécystectomie laparoscopique afin de déterminer le moment optimal de l’intervention. Matériels et méthodes : Nous avons réalisé une étude rétrospective, comparative et analytique incluant des patients admis pour une cholécystite aiguë lithiasique à l’hôpital Mohamed Boudiaf Ouargla, entre janvier 2024 et décembre 2025. Résultats : Un total de 119 patients a été inclus (11 patients dans le groupe précoce et 108 patients dans le groupe tardif). La prise en charge chirurgicale était majoritairement tardive (90,7%), malgré une consultation précoce chez 41,6 % des patients. Le retard opératoire au-delà de 72 heures était associé à une durée opératoire plus longue (81 min vs 68 min), des difficultés opératoires accrues avec des adhérences sévères (50 % vs 18,1% ; p = 0,05), un taux de conversion élevé (4,6 % vs 0 %) et une durée d’hospitalisation prolongée (6 jours vs 48 h ; p = 0,02) Discussion : Malgré les recommandations actuelles en faveur d’une prise en charge précoce, la chirurgie reste majoritairement tardive malgré une proportion non négligeable de patients consultant précocement, en lien avec des contraintes organisationnelles ou des habitudes de pratique. Nos résultats montrent que le retard opératoire est associé à une augmentation de la difficulté opératoire, du taux de conversion et de la durée d’hospitalisation, confirmant son impact pronostique. Conclusion : La cholécystectomie laparoscopique précoce (≤72 h) apparaîtcomme la stratégie optimale pour la prise en charge de la cholécystite aiguë.
Acute calculous cholecystitis is the most frequent complication of gallstones diseases. Laparoscopic cholecystectomy is the gold standard technique for treating acute cholecystitis. However, the optimal timing of the procedure remains controversial. The aim of this study was to evaluate the impact of surgical timing on the outcomes of laparoscopic cholecystectomy to determine the optimal timing for the procedure. Materials and Methods: We conducted a retrospective, comparative, and analytical study including patients admitted for acute calculous cholecystitis to Mohamed Boudiaf Hospital in Ouargla between January 2024 and December 2025. Results: A total of 119 patients were included (11 patients in the early intervention group and 108 patients in the late intervention group). Surgical intervention was predominantly delayed (90.7%), despite early consultation in 41.6% of patients. Surgical delays exceeding 72 hours were associated with longer operative times (81 min vs. 68 min), increased surgical difficulties with severe adhesions (50% vs. 18.1%; p = 0.05), a high conversion rate (4.6% vs. 0%), and a prolonged hospital stay (6 days vs. 48 hours; p = 0.02). Discussion: Despite current recommendations favoring early intervention, surgery remains predominantly delayed, even though a significant proportion of patients consult early, due to organizational constraints or established practice habits. Our results show that delayed surgery is associated with increased surgical difficulty, conversion rate, and length of hospital stay, confirming its prognostic impact. Conclusion: Early laparoscopic cholecystectomy (≤72 h) appears to be the optimal strategy for the management of acute calculous cholecystitis.
لمقدمة: يُعد التهاب المرارة الحاد الحصوي من أكثر المضاعفات شيوعًا للحصيات المرارية. وتُعتبرعملية استئصال المرارة بالمنظار العلاج الجراحي المرجعي لهذا المرض. غير أن التوقيت الأمثل للتدخل الجراحي لا يزال محل جدل. تهدف هذه الدراسة إلى تقييم تأثير التوقيت الجراحي على نتائج استئصال المرارة بالمنظار، من أجل تحديد الوقت الأنسب لإجراء العملية. المنهجية: قمنابدراسة رجعية مقارنة وتحليلية شملت مرضى التهاب المرارة الحاد الحصوي بمستشفى محمد بوضياف بورقلة، خلال الفترة الممتدة من جانفي 2024 إلى ديسمبر 2025. النتائج: شملت الدراسة 119 مريضًا (11 مريضًا في المجموعة المبكرة و108 مرضى في المجموعة المتأخرة). كانت المعالجة الجراحية في أغلب الحالات متأخرة (90.7%)، رغم الاستشارة المبكرة لدى 41.6% من المرضى. وقد ارتبط تأخر التدخل الجراحي لأكثر من 72 ساعة بزيادة مدة العملية الجراحية (81 دقيقة مقابل 68 دقيقة)، وارتفاع صعوبة العملية مع وجود التصاقات شديدة(50% مقابل 18.1%؛ p = 0.05)،إضافة إلىارتفاع معدل التحويل إلى الجراحة المفتوحة (4.6% مقابل 0%)، وإطالة مدة الاستشفاء (6 أيام مقابل 48 ساعة؛ p = 0.02). المناقشة: رغم التوصيات الحالية العالمية إلى التكفل الجراحي المبكر، لا تزال الجراحة تُجرى في أغلب الحالات بشكل متأخر، رغم وجود نسبة من المرضى الذين يستشيرون مبكرًا، وذلك بسبب قيود تنظيمية أو ممارسات اعتيادية. وتُظهر نتائجنا أن تأخر التدخل الجراحي يرتبط بزيادة صعوبة العملية، وارتفاع معدل التحويل إلى الجراحة المفتوحة، وإطالة مدة الاستشفاء، مما يؤكد تأثيره الإنذاري. الخاتمة: يبدو أن استئصال المرارة بالمنظار في مرحلة مبكرة (اقل من 72 ساعة) يمثل الاستراتيجية المثلى لعلاج التهاب المرارة الحاد.
