| Abstract: | Introduction : L’antibiorésistance est reconnue comme l’une des grandes menaces sanitaires du XXIe siècle. En première ligne face à ce défi, les internes en médecine jouent un rôle clé dans la préservation de l’efficacité des antibiotiques. Or, dans le contexte maghrébin, aucune étude n’a à ce jour documenté leurs connaissances, attitudes et pratiques (CAP) en matière de prescription. Cette étude comble cette lacune en proposant une évaluation ciblée auprès des internes algériens.
Matériels et Méthodes : Étude descriptive transversale menée entre Mars et Mai 2026 auprès de 112 internes issus de quatre facultés de médecine (Ouargla, Blida, Oran, Batna). Un questionnaire auto-administré structuré selon l'approche CAP a été utilisé, évaluant les connaissances ,les attitudes et les pratiques déclarées.L'analyse statistique a été faite par SPSS et Microsoft Excel.
Résultats : L'échantillon composé de 112 internes était majoritairement féminin (82 %). Seulement 29 % des internes avaient bénéficié d'une formation spécifique en antibiothérapie, essentiellement par autoformation. Le score de connaissances était globalement satisfaisant avec une bonne maîtrise des indications et des facteurs d'antibiorésistance. La lacune majeure concernait la méconnaissance quasi totale de la classification AWaRe de l'OMS par 82 % des participants. Les attitudes étaient globalement favorables, mais une tendance à la prescription probabiliste à large spectre sans documentation microbiologique était fréquemment rapportée. Les pratiques constituaient le domaine le plus préoccupant : près d'un tiers des internes se situait au niveau insuffisant, et la réévaluation systématique de l’antibiothérapie à 48–72 heures restait insuffisamment appliquée, ainsi qu’aucune corrélation significative n'était retrouvée entre connaissances et pratiques (p = 0,276), confirmant un écart réel entre théorie et application clinique (Knowing-doing gap).
Conclusion : Cette étude, première du genre au Maghreb, met en évidence une base théorique satisfaisante mais des pratiques prescriptives insuffisantes chez les internes algériens. Elle plaide pour l'intégration formelle des principes de l'Antimicrobial Stewardship dans le cursus des études médicales en Algérie, afin de promouvoir une culture durable du bon usage des antibiotiques. Introduction: Antibiotic resistance is recognised as one of the major health threats of the 21st century. As the first line of defence against this challenge, medical interns play a key role in preserving the effectiveness of antibiotics. However, in the Maghreb region, no study to date has documented their knowledge, attitudes and practices (KAP) regarding prescribing. This study fills this gap by conducting a targeted assessment among Algerian medical interns. Materials and Methods: A descriptive cross-sectional study conducted from March to May 2026 among 112 medical interns from four medical faculties (Ouargla, Blida, Oran, Batna). A self-administered questionnaire structured according to the KAP approach was used to assess reported knowledge, attitudes and practices. Statistical analysis was performed using SPSS and Microsoft Excel. Results: The sample of 112 interns was predominantly female (82%). Only 29% of the interns had received specific training in antibiotic therapy, mainly through self-study. The knowledge score was generally satisfactory, with a good grasp of indications and factors contributing to antibiotic resistance. The major shortcoming was the almost total lack of awareness of the WHO’s AWaRe classification among 82% of participants. Attitudes were generally favourable, but there was a frequent tendency towards prescribing broad-spectrum antibiotics on a probabilistic basis without microbiological evidence. Clinical practice was the area of greatest concern: nearly a third of trainees were at an inadequate level, and the systematic reassessment of antibiotic therapy at 48–72 hours remained insufficiently implemented; furthermore, no significant correlation was found between knowledge and practice (p = 0.276), confirming a real gap between theory and clinical application (knowing-doing gap). Conclusion: This study, the first of its kind in the Maghreb, highlights a satisfactory theoretical foundation but inadequate prescribing practices among Algerian medical interns. It advocates for the formal integration of the principles of Antimicrobial Stewardship into the medical curriculum in Algeria, in order to promote a sustainable culture of the appropriate use of antibiotics. مقدمة: تعتبر مقاومة المضادات الحيوية أحد أكبر التهديدات الصحية في القرن الحادي والعشرين. ويقف الأطباء الداخليون في الخطوط الأمامية لمواجهة هذا التحدي، حيث يلعبون دوراً أساسياً في الحفاظ على فعالية المضادات الحيوية. ومع ذلك، في السياق المغاربي، لم توثق أي دراسة حتى الآن معارفهم ومواقفهم وممارساتهم (CAP) فيما يتعلق بوصف الأدوية. تملأ هذه الدراسة هذه الفجوة من خلال تقديم تقييم موجه للأطباء الداخليين الجزائريين. طرق الدراسة: دراسة وصفية مستعرضة أجريت في الفترة من مارس إلى مايو 2026 على 112 طبيباً متدرباً من أربع كليات طب في الجزائر (ورقلة، بليدة، وهران، باتنة). تم استخدام استبيان ذاتي مركب وفقاً لنهج المعرفة والمواقف والممارسات (CAP)، لتقييم المعرفة والمواقف والممارسات المعلنة. وأجري التحليل الإحصائي باستخدام برامج SPSS وMicrosoft Excel. النتائج:كانت العينة المكونة من 112 طبيبًا داخليا تضم غالبية من الإناث (82٪). ولم يحصل سوى 29٪ من الأطباء المتدربين على تكوين إضافي في مجال استعمال المضادات الحيوية، وكان ذلك بشكل أساسي من خلال التعلم الذاتي. كانت درجة المعرفة مرضية بشكل عام مع إلمام جيد بمؤشرات الاستخدام وعوامل مقاومة المضادات الحيوية. كانت الثغرة الرئيسية تتعلق بالجهل شبه التام بتصنيفAWaRe التابع لمنظمة الصحة العالمية من قبل 82٪ من المشاركين. كانت المواقف إيجابية بشكل عام، ولكن تم تسجيل وصف مضادات حيوية واسعة النطاق دون توثيق ميكروبيولوجي بشكل متكرر. كانت الممارسات هي المجال الأكثر إثارة للقلق: حيث كان ما يقرب ثلث الأطباء الداخليين في مستوى غير كافٍ، ولم يتم تطبيق إعادة التقييم المنهجي للعلاج بالمضادات الحيوية بعد 48-72 ساعة بشكل كافٍ، كما لم يتم العثور على أي ارتباط ذي دلالة إحصائية بين المعرفة والممارسات (p = 0.276)، مما يؤكد وجود فجوة حقيقية بين المعرفة النظرية والتطبيق السريري (Knowing-doing gap). الخلاصة: تُبرز هذه الدراسة، وهي الأولى من نوعها في المغرب العربي، وجود أساس نظري مرضٍ؛ لكنها تشير إلى وجود ممارسات غير كافية في مجال وصف الأدوية لدى الأطباء الداخليين الجزائريين. وتدعو الدراسة إلى إدراج مبادئ «الإشراف على استخدام مضادات الميكروبات» بشكل رسمي في مناهج الدراسة الطبية في الجزائر، بهدف تعزيز ثقافة مستدامة للاستخدام السليم للمضادات الحيوية |
