| Abstract: | Les dermatoses professionnelles liées au latex représentent une problématique
fréquente chez les professionnels de santé exposés au port répété de gants. Elles regroupent
principalement les dermites d’irritation et les réactions d’hypersensibilité au latex, pouvant
entraîner un retentissement sur la santé et l’activité professionnelle des soignants.
L’objectif de cette étude est de décrire les caractéristiques épidémiologiques, cliniques et
professionnelles des dermatoses liées au latex chez le personnel soignant, d’identifier les
facteurs associés et d’évaluer leur retentissement professionnel.
Il s’agit d’une étude épidémiologique descriptive rétrospective menée sur une période de
cinq ans (2020–2025) au sein de l’Établissement Public Hospitalier Mohamed Boudiaf
d’Ouargla.
Elle a concerné les professionnels de santé exposés au latex ayant consulté pour une
dermatose professionnelle liée à l’utilisation des gants.
Le recueil des données a été réalisé à partir des dossiers médicaux du service de médecine
du travail, complétés par des questionnaires standardisés auprès des cas recensés. Sur une
population de 836 professionnels de santé, 33 cas de dermatoses professionnelles liées au
latex ont été recensés, soit une proportion estimée à 0,39 %. Les cas étaient majoritairement
féminins (86 %) et concernaient principalement les infirmiers (65 %). Un terrain atopique
était retrouvé chez 73 % des sujets.
L’exposition au latex était principalement quotidienne (84 %), associée à un lavage fréquent
des mains chez 78 % des participants. Les lésions étaient majoritairement localisées aux
mains (89 %), avec une prédominance des dermites d’irritation (41 %). Une amélioration
clinique sous prise en charge était observée chez 78 % des cas. La prévention repose sur la
réduction de l’exposition au latex, la formation des soignants, la surveillance médicale et
l’adaptation des postes de travail.
Le médecin du travail joue un rôle central dans cette démarche visant à préserver la santé
des professionnels et à maintenir leur aptitude au travail. Latex-related occupational dermatoses are a common problem in health care professionals exposed to repeated wearing of gloves. These are mainly irritant contact dermatitis and hypersensitivity reactions to latex, that can influence the health and the professional activity of the caregivers. Objective: To describe the epidemiological, clinical and occupational characteristics of latex-related dermatoses in healthcare workers, to identify the associated factors and to evaluate their professional impact. It is a descriptive, retrospective epidemiological study, spread over a period of five years (2020-2025) at the Mohamed Boudiaf Public Hospital Establishment in Ouargla. The study was about healthcare workers exposed to latex who consulted for an occupational dermatosis related to the use of gloves. Data were collected from the medical records of the occupational health service and standardized questionnaires were administered to the identified cases. We examined a population of 836 health care workers and identified 33 cases of latex-related occupational dermatoses, which represents an estimated proportion of 0.39%. The cases were mostly female (86%) and mostly nurses (65%). 73 percent of the subjects had an atopic background. Latex exposure was mainly daily (84%) with frequent handwashing in 78% of participants. Most lesions were on hands (89%) with irritant contact dermatitis (41%) being the most common. Management resulted in clinical improvement in 78% of cases. Prevention includes minimization of latex exposure, training of caregivers, medical surveillance and adaptation of workstations. The occupational health physician is a key player in this approach to safeguarding the health of workers and their capability to work. تع د الأمراض الجلدية المهنية المرتبطة بالتع ر ض للاتكس من المشكلات الصحية التي قد تواجه العاملين في مجال الرعاية الصحية، خاص ة نتيجة الاستعمال المتكرر للقفازات المطاطية أثناء ممارسة النشاط المهني .وتشمل هذه الاضطرابات بشكل أساسي التهاب الجلد الته يجي والتفاعلات التحسسية تجاه اللاتكس، وقد يكون لها تأثير على صحة العاملين وعلى ظروف ممارستهم المهنية. تهدف هذه الدراسة إلى وصف الخصائص الوبائية والسريرية والمهنية للأمراض الجلدية المرتبطة بالتع ر ض للاتكس لدى العاملين في القطاع الصحي، مع تحديد أهم العوامل المرتبطة بظهورها، بالإضافة إلى تقييم تأثيرها على النشاط المهني للمصابين. أ جريت دراسة وبائية وصفية استرجاعية على مدى خمس سنوات، من سنة 2020 إلى سنة 2025 ، بالمؤسسة العمومية الاستشفائية محمد بوضياف بورقلة .شملت الدراسة العاملين في مجال الرعاية الصحية الذين يتعرضون للاتكس أثناء عملهم، والذين تمت معاينتهم على مستوى مصلحة طب العمل بسبب ظهور إصابات أو أعراض جلدية مرتبطة باستعمال القفازات .وقد تم جمع المعطيات بالاعتماد على الملفات الطبية المتوفرة بمصلحة طب العمل، إلى جانب استبيانات مو حدة أ جريت مع الحالات المسجلة. من بين 836 ممارس ا صحي ا ، تم تسجيل 33 حالة من الأمراض الجلدية المهنية المرتبطة باللاتكس، أي بنسبة 0.39% كانت الإناث الأكثر تمثيلا ضمن الحالات المسجلة بنسبة % 86 ، بينما ش ك ل أفراد سلك التمريض النسبة . الأكبر، حيث بلغت .% 65 كما و جد استعداد تحسسي لدى % 73 من الحالات .وكان التعرض اليومي للاتكس هو الأكثر شيوع ا ، بنسبة % 84 ، وغالب ا ما ترافق مع تكرار غسل اليدين، الذي س جل لدى % 78 من المشاركين. من الناحية السريرية، كانت اليدان أكثر المناطق الجلدية إصابة، بنسبة % 89 ، وكان التهاب الجلد الته يجي هو الشكل السريري الأكثر شيوع ا ، حيث م ثل % 41 من الحالات .وبعد التكفل الطبي، لوحظ تحسن سريري لدى % 78 من الحالات. تؤكد هذه النتائج أهمية الوقاية من الأمراض الجلدية المهنية المرتبطة بالتع ر ض للاتكس، وذلك من خلال الحد من التعرض له قدر الإمكان، وتوعية العاملين وتدريبهم على طرق الوقاية، إلى جانب ضمان المتابعة الطبية المنتظمة وتكييف ظروف العمل عند الضرورة .ويؤدي طبيب العمل دور ا محوري ا في هذا المجال، بدء ا من الوقاية والكشف المبكر، وصولا إلى التكفل بالحالات وتقييم مدى ملاءمة ظروف العمل، بهدف حماية صحة العاملين والحفاظ على قدرتهم على أداء مهامهم المهنية. |
